As the saying goes, "a stitch in time saves nine". CLARE is for families who need to tackle difficult conversations and make plans early, when there is still time to reach good outcomes.

CLARE works with families to understand what is important to them as well as what is important for them.

​​CLARE assists older people, and people facing life- limiting illness or progressive and chronic conditions,  together with their families and significant others.
CLARE can include support people, carers and health professionals, service providers and community.

CLARE starts with a chat

At its heart, CLARE is a “chat over a cuppa”, a conversation with a skilled coach who can listen thoughtfully and offer common-sense support.
​In our experience, a great deal can be achieved even with this first step. 

CLARE meets you where you are

CLARE aims to offer only as much as each family needs to get back on track. Every person and every family is unique and we design each program case by case.

CLARE allows everyone involved to take the time they need to keep, or in some cases rebuild, respectful relationships whenever possible.
​
​CLARE is affordable and easy to access. There are no waiting lists and the cost is as little as $120 for families entitled to a concession rate[1].  Some services can be offered after hours and in venues that are safe and familiar to the family, even in the client’s own residence.
